We always welcome new patients to our practice. Your first visit to Oza Family Care and Wellness Center is very crucial for a long lasting new relationship. We specifically spend more time with you during your first visit, to better know you personally and to gather your medical history to personalize your care.

Common Questions
What should I bring to my first appointment?
Please arrive at least 15 min before your scheduled appointment to fill out the new patient paperwork, which is required during your first visit. Alternatively, you can also fill out these forms online or bring them with you to your appointment. You must bring a valid state or federal government issued photo ID and Insurance card, if you are insured.
Do I need a referral to make an appointment?
We are primary care physician's office. Most insurances do not require for you to get a referral to see a primary care physician. Some insurances require that you see your primary care physician first and get a referral to see a specialist. If Dr. Oza is covering your primary care physician, you may require to get a preauthorization from your PCP's office to see Dr. Oza.
How is the doctor-patient relationship different here, compared to other primary care clinics?
We strive to provide personalized health care at Oza Family Care and Wellness Center. We take time to listen to your concerns and address them until you are satisfied. We believe that our staff members will go above and beyond your expectations in providing patient-focused care. We value your believes and appreciate your trust and faith on us.
How will I pay for my care?
We take all major credit cards, debit cards and HSA cards. We do take cash however a non-cash transaction is preferred. At this time, we do not take checks. A photo ID is required to process any form of payment. A payment at the time of service is expected and we are required to collect your part of the payment in the form of co pay, coinsurance or deductible by your insurance.
What is your cancellation policy?
We have a 24 hour cancellation policy. If you will not be able to make your scheduled appointment with your medical doctor, please contact us by phone or email before at least 24 hours to avoid having to pay a cancellation fee.
How can I get a hold of my medical doctor?
We provide plenty of opportunity to you to ask all the questions you may have during your office visit. We understand that some questions may arise after you leave our office. During regular office hours, please contact our office staff to leave a message for the doctor. You will be contacted again as early as possible. After hours and weekends, A voicemail greeting will guide you specifically to leave a message for the doctor or to dial a new number for the covering physician. You must listen to this greeting completely as options will change every weekend.
How do you notify your patients of lab or test results?
We will notify you of your results by the preferred method you chose on the new patient paperwork. If not, we will call you on your mobile phone first. We may text you to contact us when you are available to talk. We also communicate securely through patient portal on www.patientfusion.com. If we do not call you within a specific period of time, please do not hesitate to contact us by phone or email.
Will I be able to get copies of my test results?
Yes, upon request, we are able to provide you with the copies of your test results. You can also access your records online at www.patientfusion.com after you sign up for your patient portal. An email address is required on file to access your patient portal, however, we do not prefer to email your results for HIPAA securities reasons.
Do you allow urgent or same day appointments?
Yes, We do have same day appointments available. However, if you are having an emergency, please dial 9-1-1.
What are your office hours of operation?
We are open from Monday to Thursday from 8:00 am to 5:00 pm and Fridays from 8:00 am to 12:00 noon. Currently, patients are seen by appointments. Walk-ins will be seen only when doctor is available in the office to see patients. Please call us or schedule an online appointment before coming to the office.
